Jack Thomas Smith, 76, of Independence, MO, passed away September 15, 2015 at Missouri Veterans Home in Cameron, MO.  Memorial Service will be held at 5:30pm on Sunday, September 20, 2015 at New Salem Funeral Home, Independence, MO.  In lieu of flowers, donations are suggested to the Alzheimer’s Association.

Jack was born October 11, 1938 in Kansas City, Kansas to Otto and Florence (Jacks) Smith.  He was retired from General Motors Fairfax plant. 

Jack was preceded in death by his parents; two brothers, Otto Smith, Jr. and Melvin Smith; and one sister, Betty Smith.

He is survived by his two sons: Jason Smith (Beth) of Independence and Jeffrey Smith (Vicki) of Lee’s Summit; five grandchildren: Britny Cantrell (Jason), Codey Smith, Peyton Smith, Cooper Smith, and Ethan Smith; and one great-grandson, Benten Smith.
